A) 199 Grandview Rd Skillman, NJ 08558 - An employee was working from a scissor lift and underneath a glass panel that was rigged and suspended by a fork truck. The load became loose and struck the employee. This condition exposed employees to struck by hazards. Incident occurred on August 12, 2023.
