On or about August 15, 2023, at the warehouse located at 4180 Saint Johns Pkwy, Sanford, FL 32771, the employer exposed employees to struck-by hazards, in that an employee was allowed to operate a powered industrial truck (Yale/Model GLC04AENUAE083/Type LP) with an expired training certification.
