29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about August 15, 2023, employees changing dies or maintaining equipment including but not limited to the RATCHET machines (20D155, 2000, 20MFX1, 20MFX2, and 20MFX3), HARDINGE machines (22LBF1, 22LBF2, 22LBF3, 22LBF4, 22LBF5, 22LBF6, 22LBF7, 22NC701, 2200,  and 22HARD), OMNIS lathes (13C1179, 13LTF1, 13LTF2, 13LHF5, 13LHF6, 12NC110, 12NC900, and 13NC906), and WADCO machines (25P3, 25p997, and 25R1299) in Mazaks (2000), CNC Lathes (2200), Lathe (1300), Omni (1300), Finishing (1700), and Assembly (2500) were exposed to the release of potential electrical (240volts-480volts), and pneumatic energy; the employer did not ensure each machine had clear and specific procedures to outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about August 15, 2023, employees operating machines in the assembly area were exposed to crushing/struck by hazards (from the pneumatic clamps) below the point of operation of the DENISON Multipress; the employer did not ensure one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards created by the pneumatic clamps.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a) On or about August 15, 2023, employees operating machines in the assembly area were exposed to crushing/struck by hazards (from the press) at the point of operation of the DENISON Multipress; the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle.  b) On or about August 15, 2023, employees operating machines in the assembly area were exposed to crushing/struck by hazards (from the press while manually holding the material directly next to press) at the point of operation of the spring and ball press;  the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle.  c) On or about August 15, 2023, employees operating machines in the stock cutting area were exposed to caught in/caught by hazards (from the dies), rotating parts (the 4 dies), and flying chips (from the material) at the point of operation of the ZAGAR INC. multi-spindles;  the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle.  d) On or about August 15, 2023, employees operating machines in the manufacturing area were exposed to the rotating parts (drill bit and chuck), and flying chips (from the material being drilled) at the point of operation of the drill press(11D974); the employer did not ensure to design or construct point of operation guards to prevent the operator from having any part of their body in the danger zone during the operating cycle.

29 CFR  1910.1200(f)(6)(ii):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical   a) On or about August 15, 2023, the employer did not ensure that tanks containing hazardous chemicals including but not limited to Broco 900, Broco 811, Broco Acid salt, Nickel chloride, Hydrochloric Acid, Boric Acid, Nickel sulfate spec 101, Crystal 301 Brightener, Crystal index 44F, Crystal HP Carrier, Crystal Air wetter Special, Sulfuric Acid 50%, Chromic Acid (flakes), Sulfuric Acid 50%, and Chrome 270 Catalyst were labeled.
