29 CFR 1910.27(a): The employer did not ensure that scaffolds used in general industry meet the requirements in 29 CFR part 1926, subpart L (Scaffolds). 29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. A) 353 Half Ace Road, Cranbury NJ A wooden platform was attached to an order picker and elevated approx. 20 feet from the ground to retrieve product from the racking system. The wooden platform had openings greater than 3 inches exposing employees to trip and fall hazards. Condition occurred on or about August 4, 2023.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. A) 353 Half Arce Road, Cranbury NJ The employer failed to report to OSHA within 24 hours an in-patient hospitalization involving an employee who was hospitalized for a work related injury. The incident occurred on August 4, 2023 with the hospital admission the same day, but the employer reported the incident on August 9, 2023. Condition occurred on or about August 4, 2023.
