29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. Location: Worksite On or about August 9, 2023, employees were not in instructed in the recognition and avoidance of unsafe conditions such overhead struck-by hazards, while conducting ground improving operations with hydraulic power auger attachment on an excavator. Actual Incident; On August 9, 2023, an employee of a different contractor was struck by a broken augur bit while observing ground improving operations.
