29 CFR  1926.20(b)(2):  Accident prevention responsibilities.  It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about August 15, 2023, at 110 North Fox River Drive in Appleton, Wisconsin, the employer did not ensure that a competent person had conducted frequent and regular inspections to correct recognizable hazards associated with the installation and use of electrical equipment.  No covers were installed on energized panelboards with exposed wires, no plate installed on light switch box, and panelboard disconnects were not identified with labels.

29 CFR  1926.403(h):  Electrical Identification of disconnecting means and circuits.  Each disconnecting means required by this subpart for motors and appliances shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  These markings shall be of sufficient durability to withstand the environment involved.   On or about August 15, 2023, at 110 North Fox River Drive in Appleton, Wisconsin, employees were exposed to potential electrical hazards while installing and using electrical equipment without the main electric panelboard disconnect switches identified with labels.  Using panelboards to power the building and to power building lighting during installation of lighting wires on a panelboard and receptable outlets.

29 CFR  1926.405(d):  Switchboards and panelboards.  Switchboards that have any exposed live parts shall be located in permanently dry locations and accessible only to qualified persons.  Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front.  However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.  Exposed blades of knife switches shall be dead when open.   On or about August 15, 2023, at 110 North Fox River Drive in Appleton, Wisconsin, employees were exposed to electrical hazards walking and working in the Electrical Room with exposed live parts in three panel boxes without dead fronts or covers installed.  An employee was working in a panel box within three feet of two panel boxes, employee was turning off and on circuit switches in the panel boxes, and employees were walking and working in the Electrical room as a path to the Equipment room, to use wall light switches, and to use a ladder to access an overhead work area.

29 CFR  1926.405(j)(1)(i): Live parts. Fixtures, lampholders, lamps, rosettes, and receptacles shall have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 8 feet (2.44 m) above the floor may have exposed parts.   On or about August 15, 2023, at 110 North Fox River Drive in Appleton, Wisconsin, employees were exposed to electrical hazards using wall light switches without the wall plates installed.

On or about August 15, 2023, at 110 North Fox River Drive in Appleton, Wisconsin, employees were exposed to potential electrical hazards while installing lighting equipment without tagging the de-energized circuit controls.  An employee was de-energizing circuits in the Main, Panel A, and Panel B boxes when terminating wires on the panelboards and installing lighting equipment.

On or about August 15, 2023, at 110 North Fox River Drive in Appleton, Wisconsin, employees were exposed to potential electrical hazards while installing lighting equipment without rendering the de-energized circuits inoperative.  An employee was turning off circuit switches in the Main, Panel A, and Panel B boxes when terminating wires on the panelboards and installing lighting equipment.
