29 CFR 1910.140(c)(20): The employer did not ensure that ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection are protected from being cut, abraded, melted, or otherwise damaged: (a) Battery Charging Area - On the Crown order selector (employer equipment number OS3), employees stored a harness by leaving it hanging from the Miller Scorpion Personal Fall Limiter self-retracting lifeline, subjecting the spring, for retracting the lifeline into the housing, to weakening, as evidenced by the fact that the lifeline did not retract fully into the housing even after removing the harness and having nothing attached to the lifeline's end, on or about August 16, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

1910.132(d)(1) The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE)(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY) 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Dog Bed Area - The Nedco Pillow Compression Machine (referred to as the Press Machine), model CM 506, serial number 7047, actuated by foot-operated switches, did not have complete machine guarding on the sides and did not have machine guarding on the rear nor on the point of operation, exposing the operator and other employees in the machine area to getting caught-in the press and heat sealer, on or about August 16, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: (a) Dog Bed Area - The two foot-operated switches for actuating the Nedco Pillow Compression Machine (referred to as the Press Machine), model CM 506, serial number 7047, did not have protection to prevent unintended operation from falling or moving objects or by accidental stepping onto the switches, on or about August 16, 2023. (b) Dog Bed Area - Employees used receptacles on a General Electrical relocatable power tap (a.k.a. power strip) to power loads, including a Husky 4.5 Gallon Portable Electric-Powered Silent Air Compressor and a tape machine. The UL listing for the relocatable power tap specifies using the device for supplying power to low amperage electronic equipment, exposing employees to an electrical hazard, on or about August 16, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
