29 CFR  1926.1053(b)(1):The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder.   a) Building exterior: An employee was exposed to a fall hazard of approximately 16 feet while using a ladder that did not extend 3 feet above the landing. The ladder was about 3 feet below the edge, on or about 8/16/23.
