29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards. On or about August 11, 2023, and at times prior thereto, a worker used a SkyJack SJ66T Aerial Lift without training, exposing the contractor to a fall hazard of over 6-feet.

On or about August 11, 2023, and at times prior thereto, a worker did not use an anchor that was rated for at least 5,000 pounds, exposing the contractor to a fall hazard of over 6-feet.
