29 CFR 1910.134(f)(2): The employer did not ensure that employees using tight-fitting facepiece respirators are fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is use, and at least annually thereafter: (a) Trueform Concrete, LLC, 220 Franklin Road, Randolph, NJ 07869 The employer did not ensure that employees required to use a tight-fitting facepiece respirator received annual fit testing. Employees performing casting and demolding activities were exposed to Respirable Crystalline Silica and required to use half-face 3M respirators with cartridges while performing work activities. The exposure is equivalent to a time-weighted average of 39 ug/m3 and 12.5 ug/m3. The sample times were 353 minutes and 323 minutes, respectively. Zero exposure was assumed for the unsampled time periods of 127 minutes and 157 minutes. Violation occurred on or about September 13th, 2023.

Employees performing casting and demolding activities were exposed to Respirable Crystalline Silica and required to use half-face 3M respirators with cartridges while performing work activities. The exposure is equivalent to a time-weighted average of 39 ug/m3 and 12.5 ug/m3. The sample times were 353 minutes and 323 minutes, respectively.
