29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Jackson Pine View Healthcare, LLC - 400 County Road B, Black River Falls, WI. On or about August 11, 2023, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2023. (a) On or about July 26, 2023, while working in the kitchen, an employee experienced heat exhaustion related symptoms and was taken to the local emergency room. The doctor provided the employee with a return-to-work date of July 29, 2023, which resulted in three days of lost time.
