29 CFR  1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).  On or about August 16, 2023, at an exterior building wall, a supported scaffold lacked guardrails at one end.  An employee did overhand bricklaying from the scaffold at heights greater than 10 feet and was not in use of fall protection.

29 CFR  1926.451(h)(1):Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  On or about August 16, 2023, at an exterior building wall, toe boards, or other protective devices, were not installed at a 2-tier supported scaffold where employees did bricklaying and were exposed to struck-by hazards from falling objects including but not limited to bricks, hand tools, scaffold planks.
