29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: At the jobsite located at 5920 Mission Drive, Mission Hills, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 12 feet from the eave of the roof to the ground in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. At the jobsite located at 5011 West 69th Terrace, Prairie Village, Kansas, employees using the portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 12 feet from the roof to the ground in that the side rails did not extend at least 3 feet above the landing surface of the roof. The side rails were leaning approximately 1 inch below the roof line.
