29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  5343 Anemone, San Antonio, TX 78253; On or about August 17, 2023, and times prior thereto, employees were exposed to a struck-by hazard while using a pneumatic air nail gun to install shingles on a roof without eye protection.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  5343 Anemone, San Antonio, TX 78253; On or about August 17, 2023, and times prior thereto, employees were exposed to a fall hazard while accessing the roof decking using a ladder where the ladder side rails did not extend at least 3 feet above the upper landing surface.
