29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On July 10, 2023, an employee was struck-by one of the outriggers of the pump truck, which resulted in an in-patient hospitalization of the employee. The employer did not report to OSHA within 24-hours the in-patient hospitalization of the employee. The event occurred at approximately 8:00AM on July 10, 2023, but was not reported to the local office of the Occupational Safety and Health Administration until August 14, 2023.
