29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked: a.) On or about August 08, 2023, at 9 Barstow Road, Great Neck Plaza, NY 11021. Employees doing bricks repairs on an existing building from a supported frame scaffold was missing three planks with a gap of roughly 18 inches. The working level was approximately 40 feet above the lower level. Abatement certification is not required pursuant to 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee were not protected by the use of personal fall arrest systems or guardrail systems. On or about August 8, 2023, Employees doing brick restoration from a supported scaffold that was missing an end rail were exposed to falls by not using personnel fall arrest systems or guardrails system.
