29 CFR 1926.800(b)(1): Safe means of access and egress to all underground construction workstations was not provided and maintained. (a) Worksite - Tunnel: On or about 8/18/2023, the employer did not ensure that employees were protected from struck-by and crushing hazards while operating and traveling to the underground worksite, with the MSV carrying approximately 112,000 pounds of cement tunnel segments and 3 employees, where 10 bolts were missing that connect the flatbed portion of the vehicle together.
