29 CFR  1910.28(b)(1)(i):(1) The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:     a) Job site, 1720 Metropolitan Street, Pittsburgh, PA: On or about August 18, 2023, the employer did not ensure that an employee working in a tree to cut it down used appropriate fall protection systems/devices where exposed to fall hazards of approximately 7 feet.

29 CFR  1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) Job site, 1720 Metropolitan Street, Pittsburgh, PA: On or about August 18, 2023, the employer did not ensure that an employee working to cut down a tree with a chainsaw used appropriate personal protective equipment including, but not limited to, leg, arm or torso protection, exposing the employee to cuts/lacerations type injuries.
