29 CFR 1915.71(b)(5): Scaffolds were not maintained in a safe and secure position:  a)	Steel America Structural Weld Shop Caisson: On or about August 07, 2023, tube and coupling scaffolding systems were not maintained in a safe and secure position in that wooden scaffold planks were loose and not secured thereby exposed employees to an 8-ft fall hazard.  b)	Steel America Structural Weld Shop Caisson: On or about August 07, 2023, tube and coupling scaffolding systems were not maintained in a safe and secure position in that the distance between wooden scaffold planks exceeded 1 inch thereby exposed employees to possible struck-by hazards resulting from scaffold poles and other items potentially falling between planks.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
