29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective in the following area(s): On or about August 23, 2023, on the east side of the Rig Manager's trailer, the path to ground from a standard 480v transformer was observed as not being complete and effective, exposing employees to electrical shock hazards.

29 CFR 1910.303(b)(1)(iv): Electrical equipment was not free from recognized hazards, such as damaged insulation. On or about August 23, 2023, on the east side of the Rig Manager's trailer, a cable from a 480v transformer was observed as cut, exposing the interior of the cable and exposing employees to electrical shock hazards.
