29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and rotating barrel/drum. WORKSHOP/ASSEMBLY DEPARTMENT: On/or about June 29, 2023, the employer did not sufficiently guard the right and left sides at the barrel/drum of the rivet assembly machines.

29 CFR 1910.212(a)(4): Revolving drum(s), barrel(s), or container(s) were not guarded by enclosure(s) which were interlocked with the drive mechanism so that the barrel(s), drum(s), or container(s) could not revolve unless the enclosure(s) were in place: WORKSHOP/ASSEMBLY DEPARTMENT: On/or about June 29, 2023, the employer did not sufficiently guard the barrels/drums of the rivet assembly machines in that the guards were not interlocked with the drive mechanism. On/or about June 29, 2023, the insufficient guarding by the employer at UTA Blind Rivet Assembly Machine #11 resulted in an employee finger tip amputation while replacing a pin at the barrel/drum.
