The employer did not ensure the size, amount, condition and method of stacking the blocking was sufficient to sustain the loads and maintain stability of the crane.  a) On or about August 23, 2023, at 1294 Valencia Street, Palm Bay, FL; in the driveway, the Grove hydraulic crane used to lift trusses did not have adequate cribbing, exposing employees to crush by and struck by hazards.

29 CFR  1926.1408(d)(2)(iii): 29 CFR  1926.1408(d)(2)(iii): For equipment with articulating or extensible booms: The uppermost part of the equipment, with the boom in the fully extended position, at true vertical, was less than 20 feet below the plane of the power line or more than the Table A of this section minimum clearance distance below the plane of the power line.   a) On or about August 23, 2023, at 1294 Valencia Street, Palm Bay, FL; in the driveway, the Grove hydraulic crane, used to lift trusses, operated within 20 feet of 13.2KV overhead powerlines, exposing employees to an electrocution hazard.

1926.1427(d)(1)(i)  The crane operator testing organization was not accredited by a nationally recognized accrediting agency based on that agency�s determination that industry-recognized criteria for written testing materials, practical examinations, test administration, grading, facilities/equipment, and personnel have been met.  a) On or about August 23, 2023, at 1294 Valencia Street, Palm Bay, FL; in the driveway, the crane operator was not certified to operate a 40-ton Grove hydraulic crane, exposing employees to struck-by and electrocution hazards.
