29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage  Location:  An extension cord powering the work site at 7 Florence Avenue crossed the street and went 100 feet east to an outdoor outlet at 4 Florence Avenue.  On 8/23/2023, five workers faced potential electrical shock and burn injuries due to an inadequately protected extension cord that traversed a road and passed through a pile of construction debris.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  Location:  An extension cord powering the work site at 7 Florence Avenue crossed the street and went 100 feet east to an outdoor outlet at 4 Florence Avenue.  On 8/23/2023, five workers faced potential electrical shock and burn injuries due to improper strain relief on a site-built extension cord.
