A) JA Neary Excavating, Inc. - 1100 Randolph Rd. Franklin NJ 08873 - The employer did not ensure a trench box system was installed/moved in a safe manner to protect workers from struck by and/or caught in between hazards. The system was attached to an excavator's bucket by a chain. The bucket moved  and placed tension on the chain while a worker had his hand on the hook. This caused the worker's finger to be amputated.   Condition occurred on July 17, 2023
