29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about 08/24/2023, in the Fiber Department, the employer exposed employees to caught in and amputation hazards, in that the machine Double Headed Small Riveter Model# 722, with Serial# 722-757 had its point of operation not guarded while employees held the ends of the ring while moving and adjusting the lever-latch aligned with the drilled holes on the ring. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
