The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person (s):   Location 36 Bay Farm Road Kingston MA 02364  On or about August 24, 2023, an employee was injured when he fell from a roof that was 7 feet 7 inches above a deck and 11 feet 2 inches from the ground level below where fall protection was not used. The employer did not have a program to conduct and document frequent and regular inspections of their worksites to ensure that workers were protected from falls.
