29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about August 25, 2023, and at times prior, maintenance personnel were exposed to caught in between hazards created by performing maintenance on a Lightning 50 Labeling System, serial numbers JN 85320-1-1 and JN 85320-1-2, without a documented procedure to adequately isolate energy sources to the machine, resulting in an employee suffering serious injuries when the machine was energized while performing maintenance.

29 CFR 1910.147(d)(1): The authorized employees did not have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before the authorized or affected employees turned off the machine or equipment: a) On or about August 25, 2023, and at times prior, workers were not provided with specific information regarding the types and magnitude of hazardous energy, potential hazards and the means to perform a full and effective removal of hazardous energy present in the Lightning 50 Labeling System serial numbers JN 85320-1-1 and JN 85320-1-2.
