29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Future Poly Tech located at 110 Pearl Street, Suite 4, Coldwater, Ohio, 45828: On or about August 18, 2023, the employer failed to ensure an energy control program consisting of control procedures, employee training and periodic inspections were developed and performed. Employees tasked with performing service and set up activities such as, but not limited to, webbing and threading of the rollers on the Battenfeld Gloucester Winder on Line 2 without the benefit of energy isolation, hazardous energy recognition training and employer evaluation, exposed employees to amputation hazards created by the in going nip points of the "nip" rollers of the winder.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Future Polytech located at 110 Pearl Street, Suite 4, Coldwater, Ohio, 45828: On or about August 18, 2023, the employer failed to ensure that the "nip" rollers on the Gloucester Engineering Co., Winder (Line 2) were effectively guarded. Employees walking/working in proximity of the the machine's unguarded nip-points in production were exposed to caught in hazards.
