29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) In the trench located in the area of Crossland Trail off of SR 70, Lakewood Ranch, Florida 34211: On or about 08/23/2023, employees were exposed to fall hazards in that, the ladder used to gain access and egress to the trench did not extend 3 feet above the trench edge.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) In the trench located in the area of Crossland Trail off of SR 70, Lakewood Ranch, Florida 34211: On or about 08/23/2023, employees were exposed to a cave in hazard, in that employees were conducting work in an excavation at a depth greater than 5 feet without using a protective system.
