29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   On or about August 24, 2023, and at times prior thereto, employees worked were exposed to cuts and bruises from sharp metal, rebar being cut and metal pieces.   Employees were not provided cut resistant gloves or a hazard assessment to ensure they were protected by required protective gloves and equipment.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   On or about August 24, 2023, and at times prior thereto, employees were exposed to struck-by, caught-in or -between, amputation and electrical hazards when performing repairs, service and/or maintenance of energized machinery and equipment. The control of hazardous energy program was not developed and used for the control of hazardous energies all machines and equipment such as, but not limited to:  1. The Bar Wiser Machine (Brand: Schnell; Model: TA/22/B); 2. The Bar Wiser Machine (Brand: Schnell; Model: 22NV2-3DFIB3); 3. The Bar Wiser Machine (Brand: Schnell; Model: Prima 12 R UL); 4. The Shear Machine (Brand: KRF M7; Model: H4002 Shear); 5. The Double Bender Wiser Machine (Brand: Schilt Engineering; Model: DBS3- 60N); and 6. The Cutting Bars Shear Machine (Brand: RMS; Model:6-11).
