29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a) On or about August 11, 2023, the employer did not ensure that all areas on the vertical machine center PCV40 #76 where hazards were created by rotating parts and pinch points were guarded to prevent injury to employees. Access areas around the machine were covered by doors, but the doors were not affixed to the equipment and could be opened during the operating cycle. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

The description in Column F did not describe the object that caused the injury or parts of the body that were affected. No abatement certification or documentation is required for this item.
