29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   90 Bliss Rd., Longmeadow, MA: On or about August 16, 2023, employees engaged in residential roofing activities more than 8 ft. above the next lower level were not protected by guard rails, safety net systems or a personal fall arrest systems.  Stevens Home Remodeling Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(B)(13), which was contained in OSHA inspection number 1484712, citation number1, item number 1 and was affirmed as a final order on October 26, 2020, with respect to a workplace located at 288 North Street Feeding Hills, MA.

29 CFR  1926.503(b)(1):The employer did not compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training:  90 Bliss Rd., Longmeadow, MA: On or about August 16, 2023, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained,  the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training.  Stevens Home Remodeling, Inc was previously cited for a repeat  violation of this occupational safety and health standard  29CFR 1926.503 (b) 1 , which was contained in OSHA inspection number 152919 , Citation 1 , Item 1  and was affirmed as a final order on 8/18/2021, with respect to a workplace located at 288 North St., Feeding Hills, MA.
