29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Location: a) Rooftop - suspended scaffold at the north side of the roof of the jobsite: On or about August 29th, 2023, employees were working from a suspended scaffold and had no access to and from the working platform. An employee climbed unto the edge of the roof and then climbed down to access the scaffold work platform about 11 feet below the roof.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: a) Rooftop: On or about August 29th, 2023, an employee was brick-pointing at the edge of the roof of the building without fall protection and was exposed to falling approximately 114 feet from the rooftop to the ground level below.
