29 CFR 1926.403(b)(1):Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: WORKSITE/ORNOQUE ROAD/WHEELERS FARMS ROAD, MILFORD, CT: On/or about August 28, 2023, the employer did not ensure that the employees who were required to walk along the sides of the trench were provided with a clear walk path along the sides of the trench where the flexible cords created slip and trip hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: WORKSITE/ORNOQUE ROAD/WHEELERS FARMS ROAD, MILFORD, CT: On/or about August 28, 2023, employees who were required to work in a trench 6' wide by 12' long by 6' deep used a ladder that extended only 26 inches above the trench wall. There was a trench box installed in the trench.
