29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): a) At 621 Oliver Rd., Montgomery, AL: On August 29, 2023 and at times prior, the employer failed to develop and implement an energy control program and training to protect employees from hazards associated with the release of stored energy when performing service and maintenance activities such as changing plates on the Heidelberg M-1000 C.A.C. Crimp Machine S/N 0H3397-7 Red, Blue, Yellow, and Black bottom and top sections.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Heidelberg M-1000 C.A.C. Crimp Machine S/N 0H3397-7 Red, Blue, Yellow, and Black bottom and top sections at 621 Oliver Rd., Montgomery, AL: On August 29, 2023, and at times prior, the employer exposed employees to caught-in hazards that the rotating blanket and plate cylinders on the paper machine were not guarded while manual plate loading on the machine is required.
