29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: At the worksite located at 14300 NE 41st Street Kansas City, MO: On or about August 30, 2023, employees were exposed to shock, and or electrocution hazards. A box fan was running with an electrical cord that had exposed conduits near the employee breakroom by the inbound conveyors.

29 CFR 1910.1200(e)(2): The employer that produced, used, or stored hazardous chemicals at the workplace in such a way that employees of other employer(s) could be exposed did not ensure that the hazard communication program included all of the elements outlined in 29 CFR 1910.1200(e)(2)(i) through 29 CFR 1910.1200(e)(2)(iii): At the workplace located at 14300 NE 41st Street Kansas City, MO: On or about August 30, 2023, employees engaged in warehouse activities were exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to chemicals. Employees had access to and used chemicals including but limited to the contents of a spray bottle labeled MHE and a spray bottle labeled alcohol without access to SDSs for these chemicals or training regarding the hazards of these chemicals.
