Strain relief device was missing from one of the connections end.

On or about August 15th, 2023 Location: 1401 Inwood Ave, Bronx, NY 10452 a) Employee was exposed to a fall hazard while working around floor opening on second-floor staircase C landing without personal fall protection equipment. Floor opening was about 12 ft height from the lower level.
