29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  On or about August 30, 2023 and at times prior thereto, the employer had not developed a written respiratory protection program that included provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures, such as procedures for selecting respirator for use, medical evaluations, fit testing procedures, procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators, and training of employees in respiratory hazards, exposing employees to various respiratory hazards.

29 CFR  1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.  On or about August 30, 2023, and at times prior thereto, an electrical disconnect in the torching area of the facility was blocked by products and materials, and was not readily accessible to each employee or authorized building management personnel, exposing employees to electrical hazards.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.  On or about August 30, 2023, and at times prior thereto, the employer had not implemented a written hazard communication program that included descriptions for any of the required hazard communication criteria such as labels and other forms of warning, safety data sheets, and employee information and training would be met, exposing employees to various chemical hazards.
