29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or about August 30, 2023 and at times prior thereto, the employer had not developed a written respiratory protection program that included provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures, such as procedures for selecting respirator for use, medical evaluations, fit testing procedures, procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators, and training of employees in respiratory hazards, exposing employees to various respiratory hazards.

On or about August 30, 2023, and at times prior thereto, an electrical disconnect in the torching area of the facility was blocked by products and materials, and was not readily accessible to each employee or authorized building management personnel, exposing employees to electrical hazards.
