29 CFR  1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  (a)  Facility, ERS Shop:  On or about August 27, 2023, an employee was not wearing a FR rated jacket, FR rated neck protection nor face protection while he was walking past a re-melt furnace.  Molten slag/steel splashed from the furnace and severely injured the employee with 1st, 2nd and 3rd degree burns.  The employee received burns on his right arm, neck, face, chest and abdomen.  (B)  Facility, ERS Shop:  On or about August 30, 2023, an employee was not wearing any FR rated neck protection while he was exposed to flying sparks as he was pouring slag into a re-melt furnace.
