29 CFR  1926.20(b)(1): A safety and health program was not initiated and maintained to provide compliance with the general safety and health provisions of the standard:  a) Jobsite located at 811-13 Unruh Ave., Philadelphia, PA: The employer failed to maintain a safety and health program to address the associated hazards that employees may be exposed to, such as but not limited to: fall protection and prevention, identification and control of hazards, training and education, first aid, medical training and assistance, and regular site safety inspections.   BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1571276 citation number 1, item number 2, and was affirmed as a final order on 7/15/2022, with respect to a workplace located at 2508 E. Cumberland St. Philadelphia, PA.  BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1605776, citation number 1, item number 1, and was affirmed as a final order on 1/4/2023, with respect to a workplace located at 1370 Germantown Ave. Philadelphia, PA 19123.  BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1612691, citation number 1, item number 1a and was affirmed as a final order on 1/4/2023, with respect to a workplace located at 1370 Germantown Ave Philadelphia, PA 19123.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  a) Jobsite located at 811-13 Unruh Ave, Philadelphia, PA. - On or about August 28th, 2023, the employer did not ensure that an employee working on the 2nd level of a building under active construction was protected from fall hazards by the use of a guardrail system, safety net system, or personal fall arrest system.  BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1571276 citation number 1, item number 4, and was affirmed as a final order on 7/15/2022, with respect to a workplace located at 2508 E. Cumberland St. Philadelphia, PA .  BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1598100, citation number 1 item number 1 and was affirmed as a final order on 10/13/2022, with respect to a workplace located at Buck Rd and Stoneyford Rd Northampton, PA 18067.  BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1613974, citation number 2, item number 1 and was affirmed as a final order on 3/23/2023, with respect to a workplace located at 234 E. Butler Pike Chalftont, PA 18914. 29 CFR  1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at 811-13 Unruh Ave, Philadelphia, PA. - On or about August 28th, 2023, the employer did not provide effective training for each employee installing decking on the 2nd level of a residential building, and exposed to fall hazards of approximately 20 feet, in the recognition and avoidance of fall hazards.  BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1605776, citation number 1, item number 1b, and was affirmed as a final order on 1/4/2023, with respect to a workplace located at 1370 Germantown Ave. Philadelphia PA 19123.  BCG Contractor Group was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1612691, citation number 1, item number 1b, and was affirmed as a final order on 5/9/2023, with respect to a workplace located at 1307 Germantown Ave. Philadelphia, PA 19123.
