(b)	On or about August 28, 2023, 157 Kings Row Street, Carbondale, CO the employer did not provide training to employees regarding the safe operation of equipment in that an excavator that was in operation struck a visible gas line that was above grade.  This condition exposed employees to fire and struck-by hazards.
