29 CFR  1910.22(a)(1):29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  On or about August 30, 2023, combustible wood dust accumulated on various horizontal surfaces when process equipment nearby generated a significant amount of dust particles during woodworking operations. The layers of dust that accumulated in these nonseparated dusty areas were greater than an 1/8" thick and covered more than five percent of the floor area. Bulk samples collected from the surface of equipment stored in this area confirmed that Kst explosibility was at 7.65 bar*meter/sec with an explosion severity of 1.16 (Class II Division 1). The production tools that were responsible for generating this dust included the following rough mill and finishing tools used throughout the shop:  a) The First and Second 120V BOSCH 2.25 HP Fixed-Based Routers along the west exterior wall. b) The Bi-Glass Router Machine in the Lead Room. c) The DWE6423 120V Random Orbit Palm Sanders at EE workstations. d) The SawStop Industrial Cabinet Saw Table Saw in the middle of the shop floor. e) The Bilt Hard 4.3A, 6 Inch Disc and 4 x 36 Inch Belt Sander next to the second router. f) The 15-Amp Makita 10" Dual-Bevel Sliding Compound Miter Saw next to the belt sander. g) The RYOBI 15-Amp 10 in. Sliding Compound Miter Saw across from the table saw. h) The RIDGID Shop Vac attached to the orbital sanders used at employee workstations. i) Type 2 Delta Machinery 50-760 Dust Collector (SN: 0103066 SF50).

Woodshop Floor:  On or about August 30, 2023, the employer did not ensure that the workroom floor was kept in a clean condition as combustible wood dust was allowed to accumulate on the floor, exposing employees to fire hazards.

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:   a) Woodshop Floor: On or about August 30, 2023, employees used a SawStop Industrial Cabinet Saw table saw without a self-adjusting guard while feeding stock into the saw to make straight cuts on window frames that were being restored.

29 CFR 1910.213(c)(2): Hand-fed circular ripsaw(s) were not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator:  a) Woodshop Floor: On or about August 30, 2023, the employer failed to ensure that the included spreader for the SawStop Industrial Cabinet Saw table saw was installed so that the appropriate support for the blade assembly and kickback prevention was engaged while employees used the machine to feed stock into the saw during through-sawing operations.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about August 30, 2023, the following deficiencies were found in the woodworking area:  A. Router 1: On, or about, August 30, 2023, an extension cord and a relocatable power tap was used to energize a router. The relocatable power tap was not suitable for the condition of use in a hazardous classified location.   B. Table saw: On, or about, August 30, 2023, an Industrial Cabinet Saw table saw cord was energized through a yellow extension cord. The receptacles that were not grounded did not provide a suitable condition of use in a hazardous classified location.   C. Dust Collector: On, or about, August 30, 2023, a Delta Machinery Dust Collector was energized through a yellow extension cord that was energized by another yellow extension cord. The receptacles were not grounded, and the connections did not provide a suitable condition of use in a hazardous classified location.   D. Dust Collector: On, or about, August 30, 2023, a dust collector was provided for use with the table saw to collect combustible dust funneled out through the bottom of the table saw directly into the collector. The 120V motor was provided on top of the dust intake flange of the dust collector. The motor was not enclosed to prevent the entry of dust into the windings.  E. Miter saw: On, or about, August 30, 2023, a RYOBI 15-Amp 10 in. Sliding Compound Miter Saw, across from the table saw, was energized through a red extension cord. The receptacles that were not grounded did not provide a suitable condition of use in a hazardous classified location.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a) Shop floor, table saw workstation located near the exit: On, or about, August 30, 2023, the following equipment did not have a continuous path to ground:  1. The Industrial Cabinet Saw table saw that was energized through an extension cord was not connected to a grounded metal permanent wiring system or to a system having an equipment-grounding conductor.  2. The Delta Machinery Dust Collector was energized through an extension cord that was not connected to a grounded metal permanent wiring system or to a system having an equipment-grounding conductor.
