29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Plate Shifter - The employer permits employees to perform servicing and maintenance on the plate shifter, such as end-of-shift cleanup, without utilizing the employer's energy control procedure, on or about August 31, 2023.  b) Plate Shifter - Employees perform troubleshooting on the plate shifter, such as lubricating parts and/or clearing jams, and the employer did not ensure that energy control procedures were developed, documented and utilized to control electrical and hydraulic energy sources, on or about August 24, 2023.   Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    a) Plate Shifter - The employer did not ensure employees engaging in servicing and/or maintenance on the plate shifter, such as but not limited to, end of day cleanup, troubleshooting, lubrication, and clearing jams, received training at the authorized level that included the types and magnitude of energy sources in the workplace, and the methods and means necessary for energy isolation and control, on or about August 31, 2023. 29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) Machine Room and Plate Shifter Area - The Masa block press and the plate shifter were each protected by zoned perimeter fencing and interlocked gates. An opening in a wall separating these machines was large enough for a person to pass through, which could bypass the safeguarding of each space, on or about August 24, 2023.   No abatement certification or documentation required.  Techo-Bloc Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.212(a)(1), which was contained in OSHA inspection number1587314, citation number 1, item number 3, and was affirmed as a final order on (date), with respect to a workplace located at 8111 31st Street West, Rock Island, IL 61201.
