29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). Toll booth tunnel, Rochester toll plaza - On or about August 28, 2023, the employer did not perform adequate inspections of the jobsite and materials where the HVAC ductwork was not inspected for structural stability as it was being removed, resulting in a collapse of the ductwork. An employee suffered serious injuries when they were pinned under the ductwork.
