29 CFR  1910.101(b):"Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. Section 3.3.1 of the CGA Pamphlet P-1-1965 refers to storing cylinders in accordance with CGA and NFPA standards. The CGA Pamphlet P-1-1991 Section 3.7.4.1 states that "All compressed gas cylinders in service or in storage shall be secured to prevent from falling."  NBVC Lanes Kitchen: In front of the kitchen electrical panel, two compressed  nitrogen cylinders were upright and unsecure from displacement. The cylinders were directly in front of an electrical panel blocking its access and was used as an apron rack.   Employees were exposed to struck- by and electrical shock hazards.

On and/or before August 28 2023, employee(s) used safety glasses/goggles to protect their eyes from chemical splash during maintenance activities. The employer did not ensure that the eye protection used was in working condition and free of defects such as stretched out band and dirt obstructing employee's view. 29 CFR  1910.308(b)(2): At this establishment the employer failed to ensure that all required means of egress lighting, illuminated exit signs, and all other lights necessary to provide illumination were maintained in operable condition(s).  Where emergency lighting was necessary, the system was not arranged so that the failure of any individual lighting element, such as the burning out of a light bulb, cannot leave any space in total darkness.  Back of bowling alley: On and/or before August 28, 2023, the employer did not ensure that exit signs and emergency lighting were in operable condition for the safe egress of employees.
