29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At a worksite located at 2917 Guinotte Ave, Kansas City, Missouri. Employees in the bisqisquick packing area working on line 1 and 2 were exposed to an unguarded conveyor belt with rotating parts that was missing a portion of the fabricated guard.

Employees in the bisqisquick packing area working on line 2 were exposed to serious injuries to the fingers and hands, in that the Sprocket Wheels and Chain on the located 30 Inches above the floor was not enclosed and guarded.

29 CFR 1910.303(b)(1)(iv):Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: At a worksite located at 2917 Guinotte Ave, Kansas City, Missouri. Employees charging the EZGO Textron Workhorse electric golf cart were exposed to serious injuries due to electrical shock, fire, or burns, in that electrical flexible cords were found to be in disrepair with damaged or missing outer insulation, abrasions, and splices made with electrical tape.
