29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices 2009 Edition: a. Site- A worker directing traffic was not protected from struck by hazards where the temporary traffic control plan and devices in place did not conform to Part VI of the Manual on Uniform Traffic Control Devices 2009 Edition. The worker directing traffic was not wearing high-visibility clothing; was flagging from within the traveled way; was not using a stop/slow paddle, flag, or automated flagger assistance device; and advance warning signs, including lane closure signs, were not in place. b. Site- Employees standing in at the edge of a cross walk while assisting with temporary traffic control were not protected from struck by hazards where the temporary traffic control plan and devices in place did not conform to Part VI of the Manual on Uniform Traffic Control Devices 2009 Edition. The employees were not were not using a stop/slow paddle, flag, or automated flagger assistance device; and advance warning signs, including lane closure signs, were not in place.

Intersection of Depot Street and Canal Street- A worker directing traffic, from within the traveled way, was not protected from struck by hazards where a traffic lane was closed without the use of advance warning signs, including lane closure signs. b. Intersection of Canal Street and W Auburn Street- Employees standing in a cross walk while assisting with temporary traffic control were not protected from struck by hazards where a traffic lane was closed without the use of advance warning signs, including lane closure signs.
