29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  101 Highland Ave. Holden, MA 01507: On or about September 5, 2023, the employer had not trained employees to recognize fall hazards and on the procedures to be followed to minimize these hazards, prior to exposing them to fall heights of approximately 10 feet 5 inches during a residential construction project.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  101 Highland Ave. Holden, MA 01507: On or about September 5, 2023, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed, and the signature of the person who conducted the training.  Two employees who were performing work on a residential roof were exposed to fall hazards of 10 feet 5 inches or greater.
