29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a.   On or about June 13, 2023, Production Floor, the written energy control program did not include machine specific step by step procedures to control and lockout energy sources, for employees who may perform service and maintenance (setting up) on Scrap Trim Oscillator while equipment is energized.   A machine operator sustained a right forearm injury leading to hospitalization while attaching scrap trim from 18" slitter to rotating oscillator without equipment being deenergized.   Employees were thereby exposed to the hazards associated with mechanical motion, specifically, reciprocating moving parts.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed.  a.  On or about June 13, 2023, in the facility, employees were required to service and maintain machines and equipment, including, but not limited to, slitters, scorers, coil turners, and sheet flipper.  The employer did not conduct a periodic inspection of each energy control procedure at least annually to ensure that the procedure and the requirements of the standard were being followed.

29 CFR 1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).     a.  On or about June 13, 2023, Production Floor, an employee was exposed to machine hazards associated with moving parts and pinch-points while performing service and maintenance (setup) on 18" American National Slitter.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].   As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
