29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about August 7, 2023, following a work-related in-patient hospitalization OSHA was not contacted within twenty-four (24) hours after the employer was notified of the hospitalization. H-E-B, LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1903.39(a)(2), which was contained in OSHA inspection number 1382167, citation number 2, item number 1 and was affirmed as a final order on August 27, 2020, with respect to a workplace located at H-E-B Store #094, 2409 E. Expressway 83, Mission, TX, 78572.
